Medicaid Service Coordination
This service provides advocacy, information, and case management for individuals with developmental disabilities and their families in Manhattan, the Bronx, and Queens. It also offers help in accessing services and support such as day habilitation, medical care, residential placement, and community groups to help individuals lead meaningful lives.
